I need help for my nephew having loose stool and not being able to feel it? what can i do? or recommend me?

At first my nephew had constant constipation for a long time i would say maybe more than 1 year and my sister never actually did anything about it other than just cope with it. Then it got worse that he was not able to go to the bathroom and it turned abit painfully every time he went to the bathroom and so this is when my sister finally yeah huh reacts and takes him to the doctor.

They gave him medicine he had hemorrhoids and constipation and i guess not enough fiver so yeah they help him with the problem and all.

But now the problem is that he poops and he says he does not even feel it and so for now 2 months or so he has been pooping and he says he does not fell what it might be or what is going on hes 8 years old and well cant really explain much and sister has taken him to doctor and nothing yet except that he has not enough fiver.

i am not a doctor and i question this specially when the child is telling me he does not feel the poop coming out so i am not sure what it is or what it might be so that i can tell the doctor and maybe he can check that particular problem. and so do you think his muscle does not work in the anus properly and what is that called or if this is not the problem what can it be so that i can tell my sister so she can tell the doctor what that might be.
